We are committed to serving everyone with the utmost protection during this pandemic. In order to serve you safely we have made some adjustments. PLEASE READ THE ENTIRE LIST BEFORE SCHEDULING YOUR APPOINTMENT.

  1. We have reduced the number providers working at a time in order to maintain social distancing. 

  2. Please arrive on time, not early or late, in order to help with physical distancing.

  3. Please do not bring others into the clinic for your appointment, unless you need their assistance.

  4. We maintain a sanitized environment and we clean treatment areas before and after each patient.

  5. Currently, wearing a mask is optional, but we may change that policy as conditions change.

  6. Do not come for your appointment if you are ill, have recently been exposed to someone Covid-19 positive, or are showing any signs of illness, such as a cough, shortness of breath, or fever.

  7. Our receptionists are working from home to help you schedule appointments and answer any questions you may have.

  8. All appointments are prepaid. If you are scheduling through Schedulicity you will be asked to pay for your appointment when you schedule. If you are unable to keep your appointment due to illness, your payment will be refunded or applied to a future appointment.

 
Screening Questions

In the last two weeks (or since your last visit here) have you had any of these symptoms?

  • cough

  • shortness of breath or difficulty breathing

Or have you had at least two of these symptoms:

  • fever

  • chills

  • repeated shaking with chills

  • muscle pain

  • headache

  • sore throat

  • new loss of taste or smell

 To the best of your knowledge, have you been exposed to anyone positive for COVID-19 or the Omicron variation?
Have you traveled anywhere out of the area in the last 30 days?
 
If the answer is YES to any of these questions, we are not able to treat you at this time and if you are having the above symptoms, please see your physician as soon as possible. Your health and safety and the health and safety of our employees is our TOP PRIORITY!
